Welcome to FormKind Health. These Terms of Service (“Terms”) govern your use of our telehealth services, including gender affirming hormone therapy, hormone replacement therapy, PrEP, and urgent care visits (collectively, the “Services”). By using our Services, you agree to these Terms.

1. Eligibility

  • You must be at least 18 years old to use our Services.

  • You must be located in a state where FormKind Health is authorized to provide care.

  • You agree to provide accurate, complete, and up-to-date information when creating your account or receiving care.

2. Telemedicine Services

  • Services are delivered via secure telemedicine technology.

  • FormKind Health providers may prescribe medications when medically appropriate. Prescriptions may be sent to Olympia Pharmacy or another pharmacy of your choice.

  • Telemedicine does not replace emergency care. If you are experiencing a medical emergency, call 911 immediately.

3. Payment & Billing

  • You agree to pay all fees associated with your use of the Services.

  • Payment is collected at the time of service unless otherwise specified.

  • We do not guarantee that our Services are covered by insurance unless explicitly stated.

4. Prescription Services

  • FormKind Health may transmit prescriptions to:

  • You may request prescriptions be sent to a different licensed pharmacy.

5. User Responsibilities

  • You agree to use the Services only for lawful purposes.

  • You agree not to misuse telemedicine technology (e.g., creating false accounts, providing false information).

  • You are responsible for maintaining the confidentiality of your login credentials.

6. Privacy

  • Your use of our Services is also governed by our Privacy Policy.

  • We comply with HIPAA and protect your health information as required by law.

7. Limitation of Liability

  • FormKind Health is not liable for damages arising from your use of the Services, except as required by law.

  • We are not responsible for issues related to internet connectivity, third-party software, or pharmacy processing delays.

8. Termination

  • We may suspend or terminate your access to the Services if you violate these Terms.

  • You may stop using the Services at any time.

9. Changes to These Terms

We may update these Terms from time to time. Updates will be posted on our website with a new effective date.
