How do I start care with FormKind Health?
Start by completing the secure intake form. After your intake is reviewed, FormKind Health will determine next steps, including labs, care planning, and prescriptions when clinically appropriate.
Is care completely virtual?
Yes. FormKind Health primarily provides virtual and asynchronous care. This means much of your care can be handled securely online without needing a traditional in-person visit.
What states does FormKind Health serve?
FormKind Health currently serves eligible patients in Pennsylvania, Delaware, New York, Arizona, and Vermont. Availability may vary depending on the service and state-specific regulations.

Do I need a therapist letter to start gender-affirming hormone therapy?
No. FormKind Health uses an informed consent model for eligible patients. A therapist letter is not required to start care.
Does FormKind Health prescribe testosterone?
FormKind Health currently focuses on feminizing hormone care and does not prescribe testosterone.
How often are follow-ups needed?
Follow-up care is generally completed quarterly, especially when labs or medication adjustments are needed.
PrEP, DoxyPEP, and Sexual Health
Can I get PrEP through FormKind Health?
Yes. FormKind Health offers PrEP support for eligible patients, including clinical review, lab monitoring, and prescription support when appropriate.
Is HIV testing required for PrEP?
Yes. HIV testing is required before starting PrEP and during ongoing monitoring to ensure safe and appropriate prescribing.
What is DoxyPEP?
DoxyPEP is a prevention strategy that may reduce the risk of certain bacterial STIs for eligible patients. FormKind Health can review whether it may be appropriate for you.
Pricing and Labs
How much does membership cost?
The GAHT + PrEP membership is $99/month. The GAHT + PrEP + Urgent Care membership is $129/month.
Are labs included?
Labs are included where available through FormKind Health’s lab options. Some regions, including New York, may require a separate lab payment or alternative lab process.
Can I use my insurance for labs?
In some cases, you may be able to use insurance directly with the lab. If you are self-pay, FormKind Health may provide available lower-cost lab options when possible.
Can I cancel my membership?
Memberships are designed to be simple and transparent. You may cancel according to FormKind Health’s cancellation policy.
Prescriptions and Urgent Care
Where are prescriptions sent?
Prescriptions are sent to your preferred pharmacy when clinically appropriate.
Does FormKind Health dispense medications?
No. FormKind Health does not dispense medications or operate as a pharmacy. Prescriptions are sent to the patient’s chosen pharmacy when appropriate.
What urgent care concerns can be treated virtually?
FormKind Health may be able to help with common low-acuity concerns. Emergency symptoms, severe pain, chest pain, shortness of breath, neurologic symptoms, or symptoms requiring in-person evaluation should be handled through urgent in-person or emergency care.
