FormKind provides online gender-affirming hormone therapy care, PrEP management, sexual wellness care, and eligible low-acuity urgent care services through the Wellness Membership.

The Essential Membership is $99/month and includes GAHT, PrEP, asynchronous messaging, lab review, prescription management, and follow-up care. The Wellness Membership is $129/month and includes everything in Essential plus up to 5 eligible urgent care visits per month.

No. Membership fees do not include lab costs or medication costs. FormKind can order and review labs, but patients are responsible for any lab, pharmacy, or insurance-related costs.

Not always. Many visits can be completed asynchronously through secure forms and portal messaging. A video or phone visit may be recommended if clinically needed, required by state rules, or preferred by the patient.

Asynchronous care means you can complete your intake, answer clinical questions, message your provider, receive lab orders, and receive next steps without needing to schedule a live appointment for every issue.

GAHT care may include initiation or continuation of hormone therapy, lab ordering, lab review, prescription management, refills, dose adjustments, and follow-up monitoring when clinically appropriate.

Yes. PrEP management may include risk assessment, lab ordering, HIV and STI monitoring, prescription management, refill support, and follow-up care.

Eligible urgent care concerns may include UTI symptoms, sinus symptoms, cold or flu-like symptoms, pink eye, minor rashes, nausea or vomiting, STI evaluation, birth control needs, and other short-term concerns that can be safely evaluated through telehealth.

Urgent care does not include emergency care, ER-level concerns, chronic care management, controlled substance management, complex specialist care, disability paperwork, or ongoing management of long-term medical conditions.

No. FormKind does not provide chronic care management for ongoing conditions such as diabetes, hypertension, heart disease, COPD, asthma requiring ongoing management, or other long-term illnesses.

FormKind is not a replacement for a primary care provider. We recommend maintaining a primary care provider for preventive care, chronic conditions, annual exams, vaccines, and in-person concerns.

Yes. Memberships are month-to-month and may be canceled anytime. Continued medication management may require active membership, appropriate follow-up, and completed lab monitoring.

FormKind is currently designed as a transparent self-pay membership model. Patients may use insurance separately for labs or medications depending on their insurance benefits and pharmacy coverage.

After completing your intake, your provider reviews your information and sends next steps through the patient portal. This may include lab orders, follow-up questions, prescriptions, or treatment recommendations.

FormKind does not provide emergency care. For chest pain, trouble breathing, severe allergic reactions, suicidal thoughts, severe pain, heavy bleeding, or any life-threatening concern, call 911 or go to the nearest emergency room.
